British Airtours 5390 had a window blowout because of improper fasteners being used, it almost caused the commander to be pulled out the window. Cabin crew held onto his legs while he lay on the aircraft's nose. Pressure hulls are serious shit. Like the Alaskan accident, this was not actually at full cabin pressure either.
