British Airtours 5390 had a window blowout because of improper fasteners being used, it almost caused the commander to be pulled out the window. Cabin crew held onto his legs while he lay on the aircraft's nose. Pressure hulls are serious shit. Like the Alaskan accident, this was not actually at full cabin pressure either.

@checkervest A faulty rear pressure bulkhead is squeaky-bum time. That has caused total losses in the past. JAL123, China 611. It's very weird to me, this isn't lego, the standards are crazy high, both in manufacturing and inspection. What is going on?

@checkervest The number one question I am sure the NTSB is asking is "what else?" Because you know this wasnt' a one-off.

Perhaps they couldn't find the fasteners because there weren't any...
